Tear trough treatment is the highest-skill injectable procedure on the face. The difference between an excellent and a disastrous result is almost entirely candidate selection.

1. Concavity is treatable; convexity is not

A hollow below the orbital rim responds to volume. A bulge caused by orbital fat pushing forward past a weakened septum does not — adding filler beneath it makes the bulge more prominent. These two presentations look similar to a patient and entirely different to a trained examiner.

2. Skin thickness sets the ceiling on what can be achieved

Very thin periorbital skin transmits colour from anything placed beneath it. In those patients even correctly placed hyaluronic acid can produce a bluish cast, and a more conservative plan — or no filler at all — is the honest recommendation.

3. Cannula technique is a safety decision, not a comfort one

The periorbital region carries genuine vascular risk. A blunt cannula displaces vessels rather than piercing them, which is why it is the default here even though it is slower.

4. Reversibility is the reason hyaluronic acid is the only appropriate product
